This division can provide certified copies of birth certificates from anywhere in Ohio and death certificates of those who passed away within the Massillon City limits, from 1909 to the present. The process takes about 5 minutes when you come into our office, which is located at 111 Tremont Avenue SW.  The cost of each copy is $25.  We accept cash or personal checks with a valid ID.

For BIRTH CERTIFICATES by mail, print and complete the Birth Certification Application and mail it with your payment to the Massillon Health Department.  For DEATH CERTIFICATES, print and complete the Death Certificate Application and mail it with your payment to the Massillon Health Department.  Accepted payments include money order, bank check or personal check made out to Massillon City Health Department. If paying by personal check, a legible copy of your driver’s license or state ID is required. Please include a stamped, return envelope.

Beginning January of 2011, Ohio Department of Health implemented a web-based program in which any local health department can issue a certified birth certificate originating from anywhere in Ohio. This makes it possible to get your birth certificate from your local health department, wherever you live in Ohio!

For your convenience, you can request birth and death certificates online at a trusted website: There is a service fee attached and they accept all major credit cards. Certificates ordered this way are generally mailed out the next business day. Please note that on the Shipping section, United Parcel is the delivery method checked. This is an extra fee for faster delivery. You may change that selection to regular mail if you like. Vitalchek can be used to order certificates from most parts of the country.

We do not have death records for deaths that occurred at Doctors Hospital or nursing homes and hospitals located outside the Massillon city limits. Please contact Stark County Health Department at 330-493-9904, ext. 0, for further info. These records may also be ordered online at In the Event Location section, put the township name (Perry, Jackson, etc.) instead of Massillon, so the request goes to the proper health department for processing.

Research is available for Massillon records from 1889 to the present. Information may be taken from the records or certified copies may be purchased for $25 each.

As of January 2011, birth records for all of Ohio can be obtained at any local health department.  For genealogy purposes, it is still advisable to go to the health department in the jurisdiction where the birth occurred, and you may look at the record.  Death records are filed in the health department of the jurisdiction where the death occurred.
